The Diabetes Awareness & Prevention Program is a community health initiative focused on increasing awareness, promoting early screening, and supporting healthy lifestyle choices to reduce the risk and impact of diabetes.

Diabetes is one of the fastest-growing chronic conditions in Canada and globally, and many people remain undiagnosed until complications arise. This program closes that gap through education, outreach, and accessible screening.

Diabetes Screening Clinics

Blood sugar testing, risk assessments, and referral guidance with healthcare professionals.

Healthy Living Education

Meal planning, label reading, portion control, and culturally adapted healthy choices.

Senior Wellness Outreach

Community-based awareness, glucose screening drives, and medication support for seniors.

Youth & School Engagement

Awareness presentations, healthy-lifestyle challenges, and family activities in schools.

Community Health Partnerships

Collaboration with clinics, Diabetes Canada, pharmacies, and faith & cultural groups.
